Presentation: Human Error... Why do we make all these mistakes?
Presented by: Dan Hintz

So much of the literature and training on quality improvement tends to focus on the technical. We rely on statistics, mathematical modeling, procedures, auditing, and lots of lots of forms to give us the perception that we are "controlling" or "assuring" quality. How often though, in your own experience has it come down to "well, I guess it was just human error…and what can you do about that?" We know more about our machine capabilities than we do about our people capabilities. Sometimes, our decisions and the choices we make are affected by what we think we see and what we don't see as much as by what is right in front of us. This presentation will describe a few sources of human error that you might not have thought of.
Dan Hintz is a Quality Engineer at Woodward. He has worked in quality assurances roles for 30 years as an inspector, quality engineer, manager, and master black belt. He is an ASQ certified Quality Engineer and ASQ certified Six Sigma Black Belt. He has taught classes at Rock Valley College in metrology, statistical process control, and design of experiments.
